The first SM Megamall was opened to the public on May 21, 1991, after construction on December 11, 1990. No one was hurt, but several shops were destroyed. An investigation determined that the fire started at a toy store. Its cause remains unknown, but many believed that faulty electrical wiring resulted in sparks which ignited some cleaning chemicals being used by workers in the store. Two weeks after the incident, SM had all electrical wiring checked and repaired.

The Mall is equipped with nine high-speed passenger lifts which can carry 30 persons each or 900 persons per hour. Each lift can be used as an alternative escape route in emergencies.
